Liability Waiver1. Assumption of Risk I understand that participating in outreach activities through Underground 58 Ministries may involve various physical and emotional challenges, including but not limited to: travel, exposure to varying weather conditions, interactions with members of the public, and potential hazards in unfamiliar environments. I voluntarily assume all risks associated with my participation. 2. Medical Treatment Authorization I authorize Underground 58 Ministries to provide or seek medical treatment as deemed necessary in case of injury or illness during my participation. I agree to be responsible for any and all costs incurred. 3. Release and Waiver of Liability I hereby release and hold harmless Underground 58 Ministries, its officers, directors, employees, volunteers, partners, and agents from any and all liability, claims, demands, and causes of action whatsoever arising from or related to any loss, damage, injury (including death), or illness that may be sustained while participating in ministry activities. 4. Code of Conduct I agree to conduct myself in a manner consistent with the values and mission of Underground 58 Ministries. I will show respect, compassion, and integrity while serving, and I understand that failure to do so may result in dismissal from outreach activities.
